Case Manager/Certified Peer Support Worker (CPSW)

Job Description:
This is a community-based position that serves youth and adult residents of the Pueblo of San Felipe. The Case Manager/CPSW will work in conjunction with behavioral health clinical staff to provide culturally-based life skills services to clients of the San Felipe Behavioral Health Program which meets the requirements of goals and objectives of the Pueblo of San Felipe Administration, Indian Health Service P.L. 93-638 contract, and other federal grant funding requirements. The incumbent will work under the direct guidance of the Behavioral Health Director.
Duites:
- Assist BHP clinical staff in various settings to connect with and provide case management and/or Peer Support services to clients. May be assigned to any Behavioral Health program (Schools, Village Clinic, Equine Program) as determined by supervisor
- Assist BHP staff with completing preliminary documentation for new clients. This includes consents for treatment, client right and authorizations for release of information
- Assist with warm hand-offs to Behavioral Health Program for clients seeing at San Felipe IHS medical clinic as needed
- Provides administrative and front desk support as needed and determined by supervisor
- Assist in providing client transportation and or accompany clinical staff for home visits
- Travel with staff to assist in transporting clients to and/or from treatment facilities.
- Assist in coordination of care for clients to help meet their clinical treatment needs
- Provides direct Peer Support Services to clients of the Behavioral Health program through an assigned caseload and joint services with clinical staff
- Provides direct service to clients through the CCSS framework
- Document services through electronic health records
- Participates in weekly supervision
- Provides cultural and language support to clinical staff
- Provides outreach services to current clients and other community members as determined
- Assist and guide clinical and support staff in providing culturally competent services to San Felipe members
- As “Essential Staff” may be required to be available for on-call duty coverage and must be prepared to work on days determined to be Tribal Holidays or Administrative Leave.

Qualifications:
Education: HS Diploma or equivalent preferred.
CPSW Certification: Current active CPSW certification. If not certified, incumbent will obtain CPSW certification within six months of hire
Experience/Basic Knowledge:
- Minimum one year of experience of utilizing strength-based, culturally responsive case management across the life span
- Ability to communicate in the Keres language is highly preferred
- Must have knowledge of customs and mores of the San Felipe community, and have an appreciation for the importance of traditional ways of lifestyle and language.
- Must be willing to participate in on-going training necessary for certification to participate in therapeutic interventions for tribal members.
Required Licenses/Certifications: Valid New Mexico driver’s license.
Preferences: San Felipe Preference, Keres language and Veteran’s Preference.
Working Conditions:
Works in office or community setting. Sits for long periods of time and types.
