Job Description:

Incumbent is a Local Education Agency Prevention Specialist and Certified Peer Support Worker who acts as a liaison between AWARE program- and School Administrators, school staff, service providers to address academic and mental health needs of students. Incumbent is a Keres speaking San Felipe tribal member with previous experience in care management will work to empower students and families of the San Felipe community to cultivate healthy behaviors; encourage and support coordinated, systemic approaches to the prevention of suicide and other behavioral health challenges at Santa Fe Indian High- and Bernalillo High Schools.

Duites:

Assist Community Project Manager in forming the family voice that will help implement mental health policy initiatives and prevention, intervention services for school-aged children in the San Felipe community.

Assist Community Project Manager with implementation of selected EBPs for the AWARE project.

Recruit and assist families with navigation of natural, helping networks and peer supports.

Assist with the development of recovery/resiliency plans, crisis management plans and maintain care coordination records as needed.
Advocate for tribal youth at school and at home when deemed necessary to promote positive self-care and resiliency.
Develop partnerships with schools and school staff to coordinate intervention supports for identified high risk youth.
Follow up with referral networks and youth at risk to insure that services are being provided.
Maintain a regular dialogue of all enrolled youth, adults and partners.
Assist with direct community/school outreach communications campaigns to educate tribal youth about the need for mental health services and the natural supports that exist at all venues.
Attend mandatory AWARE, school and community meetings.
Assist in creating partnerships between tribal leaders, tribal programs, Native American organizations, other supportive organizations, and state policy makers to strengthen the AWARE Initiatives.
Facilitate culturally competent youth directed activities consistent with achieving the goals of the AWARE Program.
Assist with training activities related to the AWARE program for youth, partners and project coalition participants.
Assist the Evaluation Team with the development and administration of assessment and evaluation tools.
Assist Project Evaluator in facilitating culturally competent discussions and administering evaluation tools appropriate for tribal youth, parents, elders, and other stakeholders.
Assist with management of office functions; copying, facility reservations, equipment checkout, ordering supplies, managing communications and other relevant duties as assigned.

Qualifications:

  • Education:  HS Diploma or equivalent preferred.
  • Experience/Basic Knowledge: Job related experience is required (e.g. knowledge of statewide and local public education systems, behavioral health systems specifically serving children with mental health needs and their families. Experience in social services and/or educational program networking.)
  • Required Licenses/Certifications: Prefer a valid driver’s license as regular travel to-and-from school is expected. Obtain a Peer Support Worker certification within one year of employment.
  • Physical Requirements: Must have manual dexterity to enter and retrieve data from a computer. Ability to sit and stand for long periods. Bends and stoops regularly.
  • Personal Qualities: Ability to maintain a positive professional attitude; well-organized and detail oriented; superior customer service skills; strong verbal and written skills;
  • Preferences: San Felipe Tribal Member and Keres Speaking Preference.

Working Conditions:

Works in office and school settings. Sits for long periods of time and types. Travel to-and-from school site is expected.
