CHR Coordinator
Works in coordination with San Felipe Pueblo community health programs and Indian health services.

Job Description:
Works in coordination with San Felipe Pueblo community health programs and Indian health services: Including Public Health Nursing, Health Education, Nutrition Education, Diabetes Education, and San Felipe Health Clinic to Promote Wellness, increase community knowledge of disease prevention, assist with chronic and acute disease follow-up, overseeing the Audiology program.
Duites:
- Assist Public Nurses and San Felipe Health Clinic with interpretation of medical and health education information, delivery of medication when critical to patient care.
- Assist Community Health Program staff with case management activities including home visits under the supervision of professional staff at the appropriate level of training and paraprofessional standards.
- Provides transportation to specialty care appointment for patients with no other transportation resources are available.
- Assist community health program staff with maintaining health programs.
- Attends training educational classes to increase knowledge of medical terminology, medical records, disaster preparedness, health education including, nutrition, adult and pediatric chronic and acute disease management.
- Provides monthly annual reports to Pueblo of San Felipe Health & Wellness Department Director and Tribal Administrator, Assists Health & Wellness Director with coordination of CHR staff, Represents Pueblo of San Felipe at organizational and informational meetings with Indian Health Service, and other state, county, Federal Agencies when Health & Wellness Director is unavailable.
Additional Responsibilities: This job requires supervision of unlicensed, paraprofessional Community Health Representative staff.
Qualifications:
Education: High school diploma or general education degree (GED).
Experience/Basic Knowledge:
To perform successfully in this position and individual must be willing to learn new skills, adapt to unfamiliar routines, work evening and nighttime hours, and work in close coordination with and under supervision of professional staff.
Must be bilingual in English and Keres and knowledgeable of Pueblo of San Felipe cultural values, norms, and traditions. Must have at least two (2) years experience as CHR and must have current certification or attended the following:
1. American Heart Association or American Red Cross CPR
2. American Red Cross Basic First Aide
3. Indian Health Services Basic 100 Course
4. Indian health Service Confidentiality training
5. Defense driving course
An additional 20-hour per year of training in one of the following specialty areas is also required:
- Diabetes
- Elder care
- Health Promotion and Disease Prevention
- Required Licenses/Certifications: Must have valid New Mexico Drivers license. Must be able to pass Bureau of Indian affairs Background check.
- Physical Requirements: Requires driving and involves standing, bending and walking within the community, schools, clinics, and homes. Ability to drive a car in all types of road conditions. Lifting to assist patients in and out of vehicles or onto wheelchair van lift, carrying up to 50 lbs when necessary to set up equipment for special events, clinics and teaching programs
