Job Description:

Tribal position that performs a variety of professional nursing duties in a tribal, ambulatory care setting. Under supervision, provides skilled nursing care to consumers in their homes, clinics, or the agency office using acquired skills to identify health-related problems, adopt appropriate intervention measures and promote standard health practices. This position requires coordinating primary (IHS) and behavioral health care intervention and aftercare plans with physicians, mental & behavioral health providers, hospitals, clinics, and rehabilitation specialists.

Duites:

  • Assesses patient condition, environment, and social adaptability to determine the need for primary and behavioral health services.
  • Utilizing Standards of Nursing Practice and in accordance with Agency policies and practices, will provide direct primary, preventative and therapeutic medical care appropriate to the age of patients served to include blood pressure screenings, perform TB testing, and administer immunizations as appropriate.
  • Maintains patient medical records of nursing observations, and performance of physician’s orders.
  • Formulates and implements the plan of care for patients in a systematic and coordinated approach, with appropriate verbal or written communication with licensed medical and behavioral health care providers, e.g., administers behavioral health screenings to all patients, reviews and refers qualifying scores to appropriate provider for consultation and immediate follow-up or handoff.
  • Makes home visits when indicated to implement a care plan based on family needs, e.g., dietary restrictions for diabetic or individuals with hypertension
  • Performs screening and diagnostic tests and procedures such as lab studies, x-rays, electrocardiograms, audiograms and pulmonary function tests. Determines whether to treat the patient directly or refer to a physician or to a medical specialist, e.g., a surgeon, psychiatrist, radiologist, etc.
  • Participates in the planning and coordination of various public health clinics in order to prevent communicable diseases and promote wellness.
  • Provides direct physical and psychological care in emergencies.
  • Collaborates and coordinates services delivery closely with the Indian Health Service medical providers serving the San Felipe Community for the purposes of program promotion, education and teaching in such areas as maternal and child health, nutrition, disease prevention, communicable disease control, aging and mental health.
  • Prepares and submits various patient and operational reports as directed by director.
  • Maintains confidentiality of all records and reports maintained for the health program.

Additional Responsibilities:

  • Demonstrates computer skills necessary to organize data for essential care delivery.
  • Knowledge of and ability to use electronic health record
  • Perform other duties as needed for the Behavioral Health Integration Initiative grant.

Qualifications:

  • Education: Minimum Requirements: Bachelors of Science in Nursing (BSN) degree, Registered Nurse, and Certificate of Public Health Nursing.
  • Experience/Basic Knowledge: Verbal Communication, Health Promotion and Maintenance, Listening, Quality Management, Energy Level, Integrity, Infection Control, Informing Others, Nursing Skills, Medical Teamwork, Multi-tasking. One (1) or more years of experience in a professional healthcare setting.
  • Required Licenses/Certifications: Current and valid New Mexico RN License, current basic CPR certification, and valid New Mexico Driver license.  Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS) Pediatric, and/or Advanced Life Support (PALS) Certifications and/or Experience in Behavioral Health, and Substance Abuse
  • Physical Requirements: Must have manual dexterity to enter and retrieve data from a computer. Ability to sit and stand for long periods. Bends and stoops regularly.
  • Preferences: Experience in Public Health/Community Health nursing, Behavioral health, and/or Substance Abuse. Qualified candidate who is local to the community/reservation and veteran-preference.

Working Conditions:

Works in office and within the community. Requires long hours of standing, moving and sitting. May be exposed to infectious diseases and occupational dangers.
