Job Description:
This is a community-based, grant-funded position that serves families of students of the Pueblo of San Felipe providing prevention coordination and grant required-services in accordance with the Pueblo of San Felipe Tribal Administration and applicable federal grant awarding agency. The incumbent will serve the community through working with diabetes program and other tribal program programs/activities. Focuses on the health and well-being of persons diagnosed with pre-diabetes, diabetes and those at risk for developing diabetes in the San Felipe community.

Essential Duties:
- Develops coordinates and maintain initiatives of SDPI grant and other grants available to the diabetes program. RN duties specific to patients with diabetes, pre-diabetes or at risk for diabetes.
- Identify treatment options for community members with established diabetes or pre-diabetes as well as identify individuals who may be at risk and require preventive measures.
- Educate on self-management techniques and track treatment adherence.
- Oversee a comprehensive diabetes and chronic disease prevention and management programs, as it relates to diabetes comorbidities, including supervisory, administrative and programmatic support, including reporting to tribal, federal contracts and grants.
- Facilitates development and communication of effective program policies and procedures to employees within diabetes program.
- Performs appropriate charting and documentation including the revision of care plans for patients, in collaboration with other team members.
- Participating in diabetes program multidisciplinary team to assess and provide culturally relevant care for patients.

- Providing technical assistance with guidance in planning, implementing, and evaluation of continuing education training programs on a continuous basis.
- Responsible for diabetes education, outreach and responsible for assigning and supervising the work of the diabetes prevention and control program.
- Participating in interim and annual diabetes audits.
- Will provide case management including direct primary, preventive and therapeutic diabetic care appropriate to the age of patients served.
- Design and implement diabetes related health education campaigns, disease prevention activties and screenings.
- Participate in the planning and coordination of various diabetes management clinics in order to educate, prevent comorbidities, educate and promote wellness.
- Educate the community about available health care programs and services to improve access to care.
- Participates in community analysis in partnership with community representatives and in collaboration with other public health or health care professionals.
- May make home visits when indicated to implement a care plan and/or follow-up care.
- Preforms Covid screening, testing, and collection as required to meet staffing needs and population demands.
- Collaborates and coordinates delivery of services closely with health and wellness departments, HIS medical providers serving the San Felipe Pueblo community for the purpose of the program promotion, education, and teaching.
- Participates in continuous evaluation of program and services and performance improvement.
- Maintains confidentiality of all records and reports maintained for the health program.
- Knowledge Health Services, ISDEAA 96-638, Federal Grants and Contract.
- Seeks out additional funding sources to support ongoing and new programming.
- Establishes relationships and rapport with both internal and external customers to promote and strengthen diabetes management and prevention within San Felipe Pueblo Community.

Qualifications:
Education: Registered Nurse, within one year receive CDCES or equivalent.
Experience/ Basic Knowledge: Licensed Registered Nurse in New Mexico, knowledge about diabetes, community level interventions, including home visits.
Required Licenses/ Certificates: Required; Valid driver’s license. Required; Current RN license in the State of New Mexico Required.
Physical Requirements: Must have manual dexterity to enter and retrieve data from a computer. Ability to sit and stand for long periods of time. Use both hands and fingers on a computer keyboard; bending at the wrists and elbow’ occasional stooping, bending at the waist, climbing stairs. Requires Driving
