Job Description:
Assures the maintenance of high-quality, professional, and cost-effective pharmacy services and is responsible for the Special Diabetes Program for Indians (SDPI) at San Felipe Health Center and Pueblo of San Felipe Health & Wellness Department Programs in accordance with the Pueblo of San Felipe Tribal Administration and Indian Health Service P.L. 93-638 contract and Special Diabetes Grant for Indians (SDPI)
Essential Duties: Pharmacy
- Performs: drug selection and ordering, compounding and dispensing, bulk compounding of pharmaceutical and stock preparations. Provides information and consultative services to medical staff on drugs, including their characteristics, dosages, indications, contraindications, adverse reactions, availability, and existing formulations. Participates in teaching programs for pharmacy technician students and pharmacy students.
- Develops: reviews and/or revises the policies and procedures for the pharmacy every two years. Determines that appropriate policies and procedures are being followed with the various pharmacy activities, maintains contacts with related professional services and associations. Maintains a current formulary. Revise and update the formulary through the Pharmacy & Therapeutics Committee. Participates with other professional medical staff in determining drug policy for San Felipe Health Center and Pueblo of San Felipe Health & Wellness Programs.
- Supervises ordering, stocking, and maintenance of inventory of medications including controlled substances from appropriate suppliers. Maintains all physical resources in a clean operational state. Manages medications to maintain an adequate inventory while minimizing inventory costs. Stores all medications under appropriate conditions to ensure product potency and integrity. Returns outdated, overstocked, or damaged medications to the Primary Vendor or Reverse Distributor as appropriate.
- Oversees: assists with third-party billing activities for the pharmacy department, including point-of-sale pharmacy billing for Medicare, Medicaid, and private insurance. Submits required information, including updates, to Pharmacy Benefits Managers and insurance companies for pharmacy participation in Medicare, Medicaid, and Private Insurance Plans. Coordinates with other work units to resolve problems pertaining to the online POS system.
- Assists the Pueblo of San Felipe with acquiring and maintaining New Mexico Pharmacy Certification required for Medicaid and Medicare billing. Maintains records of compliance with required Waste, Fraud and Abuse trainings.
- Develops: Monthly work schedule, monthly and/or quarterly program reports, and annual reports. Submits IHS WebCident reports for potential medication error and medication error reporting. Completes adverse drug reaction reporting, including entry into RPMS/EHR. Performs medication use evaluations to assess the appropriateness, safety, and effectiveness of medications (including those that may be high-risk, high-cost, and/or problem-prone) and identify opportunities for improvement. Assists with the development of community assessments.
- Enforces safety policies and procedures. Informs staff of potential job hazards and how to avoid them. See that all pharmacy employee injuries are reported as required. Determines the need for safety equipment. Secures the department resources from loss.
- Provides: education on medications/vaccines in community settings such as community meetings, classes and trainings for Pueblo of San Felipe employees and San Felipe Health Center staff.
- Maintains RPMS drug file, as necessary. Assists with the management of the Information Technology Access Control (ITAC) for RPMS/EHR access of Pueblo of San Felipe Health & Wellness employees. Participates with the New Mexico Board of Pharmacy Prescription Monitoring Program. Participates with the New Mexico Statewide Immunization Information System (NMSIIS) and New Mexico Vaccine For Children (VFC) Program.
Essential Duties: Special Diabetes Program for Indians (SDPI) Program
- Responsible for the SDPI Program by coordinating activities, programs, and resources for the Special Diabetes Program for Indians (SDPI) to ensure that goals and objectives are accomplished.
- The incumbent will serve the community through working with the diabetes program and other tribal programs/activities. Focuses on the health and well-being of persons diagnosed with pre-diabetes, diabetes, and those at-risk for developing diabetes in the San Felipe Pueblo community.
- Facilitates development and communication of effective program policies and procedures to employees within the Diabetes Program
- Responsible for providing diabetes patient education, monitoring of medications and disease state, and medication management and case management services in an integrated ambulatory care center; serves as the key person in the care of patients diagnosed with diabetes by providing individualized diabetes self-management education and training to individuals and groups according to the Scope and Practice, Standards of Practice, and Standards of Professional Performance.
- Develops, coordinates, and maintains initiatives of the SDPI grant and other grants available to the Diabetes program
- Develops, implements, and evaluates individual patients’ care plans in coordination with physician orders within the scope of their licensure, training, and/or Collaborative Practice Agreement. This will include medication management for Advanced Practice Providers.
- Counsels and instructs the patients and their family in the management of diabetes; encourages them to achieve diabetes self-care management.
Additional Responsibilities
Acts as a liaison between the Pueblo of San Felipe and the San Felipe Health Center staff, other health care professionals, community organizations and schools, Indian Health Service, and other State, County, and Federal agencies.
Qualifications:
- Education: Must either have a four-year Bachelor’s degree and/or a Doctorate degree in Pharmacy recognized by the American Council on Pharmaceutical Education. Incumbent must have a valid New Mexico Pharmacist license.
- Experience/Basic Knowledge: Professional knowledge of the theories, principles, practices, and techniques of clinical and basic pharmacy to provide direct clinical-pharmaceutical patient services.
Knowledge of IHS Pharmacy Standards of Practice.
Knowledge of the customs and mores of the San Felipe community, and an appreciation for the importance of traditional ways of life and language, are desired.
Demonstrates ability to communicate effectively both verbally and in writing with a wide scope of people, including Indian Health Service employees, paraprofessionals, tribal groups, government agencies, private agencies, patients, and families - Required Licenses/Certifications: Must have a valid New Mexico Driver’s license and must have a valid New Mexico Pharmacist license, BLS Certified.
- Within a year, receive CDCES or equivalent
- Physical Requirements: The overall physical demand rating for a job of the Pharmacist falls within the Light classification according to the Dictionary of Occupational Titles. Exerting up to 20 pounds of force occasionally and/or 10 pounds of force frequently, and for a negligible amount of force constantly to move objects.
- Preferences: San Felipe Preference, NA/AI, Veteran, or most qualified.
Working Conditions:
- Clean, well-lighted, and well-ventilated areas. Spend most of the workday standing. When working with sterile or hazardous pharmaceutical products, required to wear gloves and masks and use other special protective equipment.
- Position Deemed Essential. May be required to work after hours, on Saturdays, and on Tribal and administrative days.
Supervision and Guidelines:
The Chief Pharmacist is directly supervised by the Clinical Services Director.
Is responsible for coordination and professional supervision of licensed and non-licensed pharmacy program staff, including pharmacy technicians and diabetes case managers.
