Job Description:

This position is located in San Felipe Pueblo between Albuquerque and Santa Fe serving a community of approximately 4000 in accordance to the requirements, goals and objectives of the Pueblo of San Felipe Administration, Indian Health Service P.L. 93-638 contract and other contract requirements. Incumbent will provide nursing and education services to prevent, treat, manage families and groups in community homes. Provide and serve the community through work with other tribal programs/activities, home visits, epidemiological program management, infectious disease investigation and community assessment skills. Incumbent may provide direct patient services at the school clinic located on site at the San Felipe Elementary School, Head Start, and other community locations to ensure essential medical services are provided to the school age population of the Pueblo of San Felipe.

Essential Duites:

Will provide direct primary, preventive and therapeutic medical care appropriate to the age of patients serve.

  • Provide direct health care services, behavioral health screening and medication delivery.
  • Assess and coordinates medical and behavioral health care interventions and plans.
  • Collaborate and coordinate patient care services with Indian Health Service (I.H.S.) medical providers serving the San Felipe Community.
  • Make home visits when indicated to implement a care plan and/ or follow-up care.
  • Conduct assessments and screenings.
  • Undertake a comprehensive and accurate nursing assessment of clients using suitable assessment tools underpinned by evidenced based knowledge.
  • Assess and coordinates wound care and podiatry
  • Administration of medication and immunizations under standing orders.
  • Performs medical point of care testing, non-invasive screenings and other medical procedures with-in theirs cope of practice.
  • Educate people about available health care programs and services to improve access to care.
  • Perform public health screening, testing, and collection.
  • Utilize Standards of Nursing Practice and in accordance with agency policies and practices.
  • Maintains confidentiality of all records.
  • Prepare and submits various patient and operational reports directed by the Clinical Services Director.
  • Provide direct care during physical and psychological emergencies.
  • Respond to Public health emergencies, conduct testing and provide care.
  • Participate in the planning and coordination of various public health clinics in order to prevent communicable diseases and promote wellness.
  • Communicable disease screening and treatment management and education.
  • Monitor health trends and identify health risk factors unique to tribal communities.
  • Set local priorities for health-related interventions to provide the greatest benefit to the most people.
  • Design and implement health education campaigns and disease prevention activities, such as immunizations and screenings.
  • Conduct community analysis in partnership with community representatives and in collaboration with other public health or health care professionals.
  • Develop and implement nursing policies and procedures for Health and Wellness Program.
  • Participate in continuous evaluation of program and services and performance improvement.
  • Knowledge Health Services (IHS), ISDEAA 96-638, Federal Grants and Contracts
  • May perform other duties as directed.

  • Maintain current education and standards in Nursing.
  • Demonstrates computer skills necessary to organize data for essential care delivery.

Qualifications:

  • Education: Bachelor in Nursing or graduate level nursing education.
  • Experience/Basic Knowledge: Minimum of 2 years full time experience as a Licensed Registered Nurse. Excellent verbal communication, Health promotion and maintenance experience, solid nursing skills, cultural sensitivity, initiative, active listening, quality management, integrity, knowledge of infection control, ability to teach, teamwork and collaboration.
  • Required Licenses/Certifications: Current RN license in the State of New Mexico in good standing, current basic CPR certification, and valid New Mexico Driver license.
  • Physical Requirements: Must have manual dexterity to enter and retrieve data from a computer. Ability to sit and stand for long periods. Bends and stoops regularly. Ability to utilize screening and assessment tools.
  • Preferences: San Felipe Preference, AN/NA and Veteran’s Preference.
  • Additional Qualifications: HIPAA and FERPA.
  • Ability to multi-task and perform under pressure, with a sense of grace.
  • Knowledge of basic office functions and electronic health records (E.H.R.).
  • Must be willing to learn new skills, adapt to unfamiliar routines, and work in close coordination with and under the supervision of professional staff.
  • Ability to work effectively independently and as a team member.
  • Ability to speak clearly and communicate information using a work phone, cell phone and computer is required.
  • Must be able to pass Bureau of Indian Affairs Background check. (In addition to tribal background check.)

Working Conditions:

Works in office, at School Based Health Center (SBHC), Head Start and within the community. The work environment characteristics described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Exposure to outside weather in all seasons is necessary because of required visits to homes and schools. Travel is often on unpaved roads. Exposure to infections and communicable disease is possible. May work after hours, weekends, required to attend indoor and outdoor events

DRESS CODE: The medical work environment requires maintaining a professional appearance as well as protecting staff and patients from infection.

 

Supervision and Guidelines:

Guidelines include the personnel policy and department policies and procedures.

No supervisory role.
